Какие можно ждать осложнения? Как часто они встречаются? Тромбоконцентрат готовится на сепараторе Frezenius, COBA, Amicus. Спасибо

Уважаемый Сергей! Конечно, возможны, но однозначного подхода нет. Если Вы живете в России, то нужно помнить о 363-м приказе: "Пара "донор-реципиент" при переливании тромбоцитного концентрата должна быть совместима по антигенам АВ0 и резус. Несовместимость по АВ0 снижает эффективность донорских тромбоцитов. Однако в повседневной клинической практике, особенно при большом количестве реципиентов, нуждающихся в переливании тромбоцитного концентрата, и ограниченном количестве доноров, допустимо переливать тромбоциты 0(I) группы реципиентам других групп крови, не задерживая трансфузию в поисках совместимых тромбоцитного концентрата."
В мире практикуется и такой подход: "Platelet concentrates from donors of the identical ABO group as the patient are the
components of choice, and should be used as far as is possible (Grade B
recommendation, Level of evidence IIa). Platelet recovery of both major and minor
ABO incompatible platelet transfusions may be impaired to some extent, but this is
not usually clinically significant. A more important consideration concerning donor
ABO antibodies is the avoidance of acute haemolysis, which may occur after ABOincompatible platelet transfusions, typically with transfusions of high titre anti-A to
A1 recipients. This problem has been particularly apparent in small children receiving
apheresis platelet concentrates, which contain large volumes of plasma from single
donors. Group O platelets should only be used for group A, B and AB patients if they
have been tested and labelled as negative for high titre anti-A and anti-B (Grade B
recommendation, Level of Evidence III).
RhD negative platelet concentrates should be given, where possible, to RhD negative
patients, particularly to females who have not reached the menopause (Grade B
recommendation, Level of Evidence III). If RhD positive platelets are transfused to a
RhD negative female of childbearing potential, it is recommended that anti-D should
be given (Grade B recommendation, Level of Evidence III)."
